Predict a long-term cognitive outcome of iNPH patients from change of Aβ42 toxic conformer ratio in CSF.

The research group led by Dr. Chihiro Akiba, Department of Neurosurgery, Juntendo University has reported in the following study that change of Amyloidβ1-42 (Aβ42) toxic conformer ratio in CSF with idiopathic Normal Pressure Hydrocephalus (iNPH)* patients predicts a long-term cognitive outcome.

2 cohort results are reported in this research.

The first cohort consists of Alzheimer's disease patients (AD, N = 17), iNPH patients (iNPH, N = 17) and cognitively normal control (CN, N = 12).  Aβ42 toxic conformer ratio (Aβ42 toxic conformer/Aβ42×100) in the iNPH group was significantly higher than the CN group and significantly lower than the AD group.

The second cohort consists of 51 patients with iNPH and it was divided into low level pTau (Age-adjusted) and high level one. The high level group showed cognitive decline for 2 years while the low level one showed its improvement after 1 year after shunting. 

The low level pTau group was divided into the group decreased and increased of Aβ42 conformer ratio for 1 year after CSF shunting for further investigation. In the result, the decreased group showed high ratio of improvement of cognitive capability while the increased group was observed decline the capability after 2 years. 

From this outcome, it has been suggested that a long-term cognitive outcome can be predicted from the change of Aβ42 conformer ratio in CSF with iNPH.

*idiopathic Normal Pressure Hydrocephalus (iNPH) is a disease that squeezes brain and causes disability by abnormal CSF accumulation in brain. The symptoms are included gait disturbance, dementia, and urinary incontinence. The syndrome can be improved by CSF shunting.
